Quick Depression Screen

This test is a preliminary screening test for depressive symptoms that does not replace in any way a formal psychiatric evaluation. It is designed to give a preliminary idea about the presence of mild to moderate depressive symptoms that indicate the need for an evaluation by a psychiatrist.
Have you been feeling sad...down in the dumps?
Do you still enjoy your job, sports, hobbies?
Do you often feel tired?
Do you have trouble sleeping or do you sleep too much?
Have you been gaining or losing weight?
Do you often feel down on yourself, that everything is your fault?
Do you have trouble making decisions or concentrating on work?
Do you often feel agitated or like you can barely move?
Do you ever feel that life isn't worth living?



Test Results:
If you checked at least five of the symptoms above, including the first and second questions (depressed mood and loss of interest in activities) and have experienced these problems for 2 weeks or longer without a physical cause, you may be depressed.
