Position Title: Case Manager I

 

Reports To:                Program Director

 

FLSA Status:             Non-Exempt

 

 

                                                                                                                                 

Nature of Work:  Position responsible for securing and monitoring client services as specified in the Individual Program Plan or as an emergency situation requires.  Acts as Westbrook’s primary contact with clients.  Provides assessment, service planning, linkage/referral, advocacy, crisis assistance, monitoring, travel and reporting services. 

 

 

 

Responsibilities:

 

1.      Documents activities and services in a complete, accurate and timely manner complying with Licensing Regulations and Westbrook policy in regard to format and content of notes.

2.      Coordinates and chairs the Interdisciplinary Team to ensure an Individual Program Plan is developed for each client and reviewed at least every 90 days.  Informs IDT members of significant changes in client(s) situation.

3.      Advocates for and assists the client in obtaining needed services.

4.      Provides crisis assistance as needed.  Performs on-call duties as required

5.      Assists client n maintaining eligibility for applicable programs, monitoring financial status, and conducting required evaluations within time lines specific to applicable program regulations.

6.      Work with staff, clients and others in a professional, cooperative, ethical, respectful and effective manner.

7.      Maintain a level of attendance consistent with Westbrook Health Services.

Minimum Requirements:

 

1.                  Bachelor’s Degree

2.                  Social Work License

3.                  Valid Drivers License

4.                  Insurance Coverage to meet Westbrook requirements

5.                  Pass CIB check

 

 

Physical Requirements:

 

Sitting:                        6 hours per 7.5 hour shift.  Alternates frequently to walking and

 standing.

Standing:         1 to 1-1/2 hrs. per 7.5 hour shift. Alternates frequently to walking and sitting

Walking:          1 to 1-1/2 hrs. per 7.5 hour shift.  Alternates frequently to sitting and standing
