JOB DESCRIPTION
Position Title: Residential Consumer Assistant II
Reports To: Residential Supervisor
FLSA Status: Non-Exempt
Funding
Source: Title XIX Waiver Program
Nature of Work: Position responsible for direct, basic and
supportive care to Title XIX Waiver clients.
Provides client supervision, training, health monitoring, personal care
and transportation for individuals in the MR/DD group homes. Provides leadership role in the overall
operation of a program component.
Responsibilities:
1. Documents activities and services in a complete accurate and timely manner according to DHHR licensing regulations and Westbrook policy.
2. Monitors clients to ensure the safety observing health and vital signs, client functional and dysfunctional behavior according to Individual Program Plan.
3. Implements Skill Acquisition and Behavior Management Programs conducting training as scheduled and recording data as training occurs.
4. Acts as job coach as needed.
5. Maintains facility to ensure sanitary conditions and general housekeeping duties.
6. Transports clients to activities, services, appointments and general community.
7. Monitors general health condition of client, performing basic medical treatments, dispensing medications as permitted by State regulations.
8. Assists in the integration of the client into the community.
9. Maintain a level of attendance consistent with Westbrook Health Services.
10. Acts as mentor to Consumer Assistant I assigning tasks ensuring schedules and programs are followed. Notifies supervisor of potential problems.
11. Works with staff, clients and others in a professional ethical and respectful manner.
12. Works
to ensure a safe environment for staff, consumers and associates by following
Westbrook’s safety policies.

Minimum
Requirements:
1. High School Diploma/GED
2. Pass CIB check
3. Valid Drivers License
4. Current Auto Insurance
