JOB DESCRIPTION

Position Title: Engagement Specialist

Reports To: Program Director

FLSA Status: Non-Exempt

Funding Source: PATH Grant

Nature of Work: PATH (Projects for Assistance in Transition from Homelessness). Provide linkage to homeless and mainstream services. Provide care for immediate needs such as clothing, food and shelter. Provide crisis intervention and referrals for immediate care.

Responsibilities:

1. Provide daily outreach to homeless clients.

2. Seek out clients in community setting.

3. Provide linkage/referral for clients with services.

4. Provide transportation to appointments and supports.

5. Maintain a level of attendance consistent with Westbrook Health Services.

6. Works with staff, clients and others in a professional ethical and respectful manner.

7. Works to ensure a safe environment for staff, consumers and associates by following Westbrook’s safety policies.

Minimum Requirements:

1. High School Diploma/GED

2. Pass CIB check

3. Valid Drivers License

4. Current Auto Insurance

Physical Requirements:

Sitting: 1 to 2 hours per 8 hour shift. Alternates frequently to walking and

standing.

Standing: 6 hrs. per 8 hour shift. Alternates frequently to walking and sitting

Walking: 6 hrs. per 8 hour shift. Alternates frequently to sitting and standing
