Position Title: Hospital Liaison

Reports To:                Mental Health Program Director

FLSA Status:             Non-Exempt


Nature of Work:  Participates in the treatment team/discharge planning for individuals committed to the hospital, scheduling appointments for post-discharge care. 




1.      Provide face to face or phone contact with individuals admitted to hospital or the assigned social worker for the individual in service area within 48 hours or two working days of admission. Contact with individuals admitted for crisis residential service should occur with 48 hours.

2.      Participates in treatment team/discharge planning meeting for individuals committed to hospital from service area.

3.      Provides information regarding availability of treatment, residential support services and community resources to treatment team for individuals committed to or diversions to hospital for the purpose of discharge planning.

4.      Coordinates scheduling of initial intake for post discharge services for discharged individuals.  Prior to discharge, schedules and documents on discharge plan initial intake for post discharge services appointment to occur within 48 hours of discharge.

5.      Coordinates scheduling of initial medical evaluation.  Prior to discharge, schedules and documents on discharge plan medical appointment to occur within 96 hours of discharge.

6.      Coordinates face to face contact with individuals discharged from hospital. Contact will occur within 24-48 hours of discharge.  Contact will be scheduled before discharge and documented on discharge plan.

7.      Coordinate with hospital to ensure discharged individual will have medication needs met until scheduled medical appointment.

8.      Work with staff, clients and others in a professional, cooperative, ethical, respectful and effective manner.

9.      Maintains a level of attendance consistent with Westbrook Health Services policy.

10.  Observes agency Risk Management policies to include Safety.

11.  Abides by agency policies regarding HIPAA regulations.

12.  Responsible for understanding and abiding by Westbrook and licensing organization Code of Ethics.



Minimum Requirements:

1.                  Associates Degree

2.                  Valid Drivers License

3.                  Insurance Coverage to meet Westbrook requirements

4.                  Pass CIB check
