Position Title: SA Tech, Amity

Reports To:                Program Director, Amity

FLSA Status:             Non-Exempt

Funding Source:        OBHS Grant, Medicaid, Rehab

 

 

                                                                                                                                 

Nature of Work:  Position responsible for direct, basic and supportive care to consumers.  Provides supervision, training, health monitoring, personal care and transportation.

 

Responsibilities:

 

1.      Documents activities and services in a complete accurate and timely manner according to regulatory agency’s  regulations and Westbrook policy.

2.      Monitors clients to ensure their safety observing health and vital signs, client functional and dysfunctional behavior according to Individual Program Plan.

3.      Maintains facility to ensure sanitary conditions and appropriate living arrangements.

4.      Transports clients to activities, services, appointments within the community and state

5.      Provides direct individual and group supportive therapy services.

6.      Maintains Medical Records Charts for program and conducts quality assurance oversight and monitoring.

7.      Answers and responds to crisis line calls.

8.      Monitors general health condition of client, performing basic medical treatments.

9.      Conducts client breathalyzer and urine screens according to program policies and procedures.

10.  Documents pre-admission  fro program on phone referrals

11.  Maintain a level of attendance consistent with Westbrook Health Services policy.

12.  Works with staff, clients and others in a professional ethical and respectful manner.

13.  Works to ensure a safe environment for staff, consumers and associates by following Westbrook’s safety policies

Minimum Requirements:

 

1.                  High School Diploma/GED

2.                  Pass CIB check

3.                  Valid Drivers License

4.                  Current Auto Insurance

 

 

Physical Requirements:

 

Sitting:                        1 to 2 hours per 8 hour shift.  Alternates frequently to walking and

 Standing.

Standing:         6 hrs. Per 8 hour shift. Alternates frequently to walking and sitting

Walking:          6 hrs. Per 8 hour shift.  Alternates frequently to sitting and standing

 

Frequency:

Never = 0%                             Rarely= 1 – 10%         Occasionally= 11 - 33%

                        Frequently= 34 – 66%                        Continuously= 67 +%
